The South African healthcare environment, with its many challenges and resources, provides opportunity for the development of innovative and cost-effective approaches to the management of the increasing burden of diabetes mellitus. In 1994, the Centre for Diabetes and Endocrinology (CDE), established a novel community-based capitation and risk-sharing model for diabetes management. From a single 'Centre of Excellence', was born a nationwide diabetes care network, which over the years, has grown to currently offer the services of 31 Endocrinologists / Diabetologists, 48 Specialist Physicians (Internists), 165 'Centres of Excellence' run by trained and dedicated General Practitioners and Family Physicians, and 610 contracted primary care doctors. This growing network effectively offers primary, secondary and tertiary levels of expertise and care nationally in the private sector and provides a comprehensive diabetes treatment and management Programme based on appropriate, best-practice, integrated diabetes care principles. Apart from intensive glycaemic control and risk factor management, this Programme promotes a holistic, individualized care approach including team-facilitation of patient self-management using educational, counselling and coaching techniques.
