Exercise testing, as a diagnostic and therapeutic modality in managing pulmonary diseases in children, has not been fully utilized. Monitoring the unique response to the stress of exercise that children possess can aid in the diagnosis of pulmonary pathophysiology. This review addresses the exercise response in pulmonary diseases and discusses the role of exercise in the continuing management of these disorders. Pediatric pulmonary disorders including asthma, cystic fibrosis, chest-wall deformities, interstitial lung disease, vocal cord dysfunction, and lung transplantation are highlighted.
