Expanding access to treatment, which we all hope is now a reality in South Africa, may bring with it new chal-lenges, specifically that of the distinction between the acute/palliative interface highlighting the fact that both 'curable' and palliative elements exist in the treatment of HIV. As with cancer, support early in the disease is impor-tant, as fears of treatment toxicity, disability and eventual death arise. In fact it is particularly important that those planning HIV care avoid viewing palliative care and dis-ease- specific therapies as an either/or ..
