CASE REPORT 22 Heterotopic pregnancy: A case report & literature review An ultrasound examination was performed and the diagnosis of a pregnancy in the left fallopian tube with free fluid in the pouch of Douglas was made. An intra-uterine pregnancy of 6 weeks gestation was also identified. The patient was taken to theatre for a laparotomy after careful counselling. At laparotomy a ruptured ectopic pregnancy was identified with a hemoperitoneum of about 300ml. The uterus was 8 weeks in size. The clots were gently suctioned from the pouch of Douglas.
