The persistence of micro-organisms within the root canal system may induce an inflammatory and immune response within the periradicular (periapical) tissues resulting in local bone destruction. Furthermore, contamination of the periradicular tissues by micro-organisms and root filling material may initiate a foreign body reaction, thereby impairing tissue healing. The most common therapeutical options for the re-treatment of teeth with periapical pathosis are non-surgical orthograde treatment and surgical treatment.
