Abstract

Assessment of a wound is not only the starting point of wound care but also an ongoing process throughout wound management. Assessment should therefore be seen as an integrated and continuous process. The most important factor to remember is that wound assessment should be done holistically - the wound is attached to a person with individual qualities and needs that should be considered. A wound can never be treated without considering the unique physical and psychological aspects of the human being.


This article gives a standard and systematic approach to wound assessment and will enable any nurse to do a basic wound assessment in order to apply appropriate treatment.
