Abstract

A European girl of 13 years presented with sustained hypertension. While in hospital she underwent a spontaneous, almost fatal, hypotensive crisis. The adrenal phaeochromocytoma subsequently removed contained epinephrine but no norepinephrine. After cystoscopic examination to exclude a further phaeochromocytoma, since no benefit had accrued from removal of the adrenal neoplasm, she sustained a second attack of hypotension which ended in death. Necropsy revealed malignant para-aortic phaeochromocytomata with invasion of sympathetic nerves and ganglia, as well as a localised argentaffinoma of the duodenal papilla. Norepinephrine only was recovered from two of these tumours.
