Abstract

The importance of atheroma in the production of ""stroke"" is stressed. The stroke may be of a temporary nature, a ""warning stroke"" resulting from embolisation of material from an atheromatous plaque, and should this arise from the carotid system endarterectomy may produce excellent results. Should the carotid artery be totally occluded the results are less satisfactory. Where the internal carotid artery is totally blocked and the blood supply to the hemisphere is greatly reduced, a by-passing shunt can be made by anastomosing the superficial temporal artery to one of the distal branches of the middle cerebral artery. Eight operations of this nature are reported, middle cerebral endarterectomy for partial occlusion of that vessel is also discussed. It is too early for critical evaluation of the results.
