An unusually young case of polycythaemia vera is presented in whom a long follow-up was available and in whom the relationship between polycythaemia vera and myelofibrosis as two components of the myeloproliferative syndrome is well illustrated. The value of leukocyte alkaline phosphatase estimations in differentiating myeloid leukaemia from the leuko-erythroblastic peripheral blood picture of myeloid metaplasia and the place of radioactive isotopic studies in defining the anaemia of myelofibrosis are discussed. The therapy of polycythaemia vera and the management of the anaemia associated with myelofibrosis and myeloid metaplasia are briefly reviewed.
