Abstract

The problem of deciding what operation to advise in chronic duodenal ulcer has been simplified by recent work. If the acid secretion is high, then it is necessary to reduce it by vagotomy. Vagotomy alone is unsatisfactory and should be combined with drainage procedure. In those cases where vagotomy does not reduce the acid secretion as tested by medical vagotomy, it is also necessary to remove the antrum which secretes gastrin in order to prevent recurrent ulceration.
