During a research problem identification session in the Uthukela District management identified low staff morale and lack of adherence to policy as major problems. At the same time it was felt that nurses were often not aware of their own mistakes, so that errors became the norm. Ideally, all these problems should be addressed by supervisors, but the job of a supervisor was not clear and therefore not being done well.
