Due to limited literature devoted to understanding coalitions and the dynamics that influence their development in South Africa, this study explores the Eastern Cape Health Crisis Action Coalition (ECHCAC), focusing on its formation, structure, coalition members’ beliefs systems, how those beliefs influence participation and growth within the coalition. The study found that belief systems influence people’s choice in partnerships and uptake of interventions. The diversity of these core beliefs can cause problems in coalitions. The challenges mentioned by the coalition members include the lack of understanding the implications of diversity, participation, communication, and significant impact within the health sector service delivery system. Stagnation, lack of cohesion and systemic learning within the coalition were further causes of concern for coalition members. ECHCAC members expressed a need to try different approaches to strengthen the coalition, including clarifying expectations, regular communication, sharing of experiences.
