Introduction: The occurrence of colorectal cancer during pregnancy is rare and is associated with diagnostic and therapeutic challenges. This is a case report of sigmoid cancer during pregnancy, and a review of the literature.

Case Report: A 38-year-old multiparous pregnant woman, in the 20th week of gestation presented with pain on both flanks which was radiating to the back, and vomiting for a period of 2 weeks. An emergency laparatomy, biopsy, and de-functioning colostomy was done. A second laparatomy resection of the sigmoid tumour and biopsy of the paraaortic lymph nodes was done. The patient is currently awaiting adjuvant chemotherapy at Steve Biko Hospital.

Conclusion: This case proves that colorectal cancer can mimic the signs and symptoms of pregnancy and turns to present at an advance stage in these patients.
